Can Vaginoplasty Help with Urinary Leakage or Pelvic Floor Issues?

This is a question we want to answer carefully, because the honest answer is nuanced — and getting it right matters. The short version: vaginoplasty can have a positive effect on certain pelvic floor symptoms for some patients, but it is primarily designed to address vaginal laxity and restore sexual sensation — not to treat urinary incontinence as its primary goal. Let us explain the distinction.

What Vaginoplasty Actually Does

Surgical vaginoplasty tightens the vaginal canal by repairing and bringing together the underlying pelvic floor muscles — specifically the levator ani and perineal muscles. These are the same muscles that, when weakened or separated, contribute to both vaginal laxity and, in some cases, pelvic floor dysfunction.

Because of this overlap, some patients do notice an improvement in mild urinary symptoms after vaginoplasty — particularly stress incontinence (leakage with coughing, sneezing, laughing, or exercise). This is a secondary benefit for patients whose incontinence is directly related to the same muscle weakness driving their vaginal laxity.

However, vaginoplasty is not a replacement for dedicated incontinence surgery, and it is not designed or indicated as a primary treatment for urinary leakage. If incontinence is your main complaint, we want to make sure we match you with the right treatment — which may or may not involve vaginoplasty.

Non-Surgical Options That More Directly Target Urinary Symptoms

For women whose primary concern is urinary urgency or mild to moderate stress incontinence, we often recommend starting with non-surgical options before considering surgery.

FemTouch Vaginal Laser uses a fractional CO₂ laser to stimulate collagen production in the vaginal walls and surrounding supportive tissue. As tissue quality improves, the urethral support structures also benefit — and many of our patients see meaningful improvement in urinary urgency and mild leakage after their FemTouch series without any surgery at all.

TempSure Vitalia uses radiofrequency energy to tighten vaginal tissue internally and externally, and has also been shown to help decrease urinary urgency and incontinence symptoms in many patients.

Both are excellent first-line options for women whose incontinence is mild to moderate and driven primarily by tissue atrophy or laxity.

When More Specialized Treatment Is Needed

For women with significant or long-standing urinary incontinence — particularly those who have moderate to severe stress incontinence that substantially affects their daily life — a referral to a urogynecologist for evaluation of dedicated surgical options (such as a mid-urethral sling procedure) may be the more appropriate path. We will tell you clearly if we believe your symptoms warrant that conversation, because we want you to get real, lasting relief.

Pelvic Floor Physical Therapy: A Valuable Partner

Regardless of which treatment path makes sense for you, we often recommend pelvic floor physical therapy as a complement to both surgical and non-surgical vaginal rejuvenation. A skilled pelvic floor PT addresses the muscular component of pelvic floor weakness in a way that no procedure alone can fully replicate. Combining pelvic floor PT with vaginoplasty or a non-surgical treatment often produces better functional outcomes than either approach alone.
